Heat a large pot over medium-high heat. Add the onion and garlic and sauté for 2 minutes until the onion is translucent. Add the spices and cook for 1 minute until fragrant. Add the clam juice, tomatoes, bell pepper, carrots, potatoes, cilantro, lemon juice, salt and pepper and bring to a boil. Reduce the heat to low and simmer for 45 minutes until the vegetables are tender.
Once the vegetables are cooked, add the fish fillets and cook for an additional 5 minutes until the fish is cooked through. Serve the stew hot with crusty bread.
